• It is possible to obtain equipment and administrative support services at a legitimate market value, pending state regulations.

Qualifications and Experience of Optometry Provider:

• An individual with a licensed degree in Optometry (OD) or Ophthalmology (MD or DO) from a recognized institution.
• Official certification allowing practice in optometry or ophthalmology.
• General Liability Certificate.
• The need for a Professional Liability Insurance Certificate has become increasingly necessary as businesses become more litigious in nature.
• W-9 Form.
