Job Summary of Independent Optometrist:

Launch your services in Sam’s Clubs! As an Independent Optometrist leasing space inside our Sam’s Club, you will take a leading role in patient care, focusing on eye and vision health as well as general well-being. As an independent leaseholder, you have the autonomy to establish your days of practice, set your hours of operation, control your patient schedule, and keep 100% of your exam/professional fees.

Job Duties and Responsibilities of Independent Optometrist:

• Establish your practice days and hours of operation.
• Control your patient schedule and fee schedule.
• Provide eye and vision care services to patients.
• Keep 100% of exam/professional fees.
• Optional: Opt into equipment/instrumentation and administrative/Opt-Tech support services (where allowed by state regulations).

Qualifications and Experience of Independent Optometrist:

• Doctor of Optometry (OD) from an accredited optometry school or Ophthalmologist (MD or DO) from an accredited school.
• Valid state optometry or ophthalmology license.
• General Liability Certificate.
• Professional Liability Insurance Certificate.
• W-9 form.
