As an independent optometrist leasing space within Sam's Club, you can have the independence to manage your own schedules and fees while providing exceptional eye and vision treatments. Working independently, you can also take advantage of additional auxiliary services.

Job Duties and Responsibilities of Eye Care Professional:

• Arrange your calendar to prioritize practice sessions during your most productive hours of the day.
• When developing your fee structure, consider factors such as the complexity of the service being provided, the time required to deliver the service, and the cost of any necessary equipment or materials.
• Keep the complete amount of your fees for your exams and professional charges.
• It is possible to obtain equipment and administrative support services at a legitimate market value, pending state regulations.

Qualifications and Experience of Eye Care Professional:

• A practitioner who has obtained a recognized degree in Optometry (OD) or Ophthalmology (MD or DO) from a reputable institution.
• An authentic and up-to-date license for optometry or ophthalmology.
• General Liability Certificate.
• Without a Professional Liability Insurance Certificate, professionals may have to face legal fees and damages out of pocket in the event of a claim.
• W-9 Form.
