Qualifications and Experience of Optometry Professional:

• A licensed individual who holds a degree in Optometry (OD) or Ophthalmology (MD or DO) from a reputable college or university.
• A valid license granted to individuals practicing optometry or ophthalmology.
• General Liability Certificate.
• The need for a Professional Liability Insurance Certificate has become increasingly necessary as businesses become more litigious in nature.
• W-9 Form.
