• Do not reduce the amount of your examination or professional charges.
• Administrative support services and equipment can be secured at a reasonable rate, subject to strict compliance with state laws.

Qualifications and Experience of Optometry Service Provider:

• A qualified medical professional with a degree in Optometry (OD) or Ophthalmology (MD or DO) from an accredited institution.
• A verified license to operate in the fields of optometry or ophthalmology.
• General Liability Certificate.
• Obtaining a Professional Liability Insurance Certificate is often required by law for certain professions.
• W-9 Form.
