• Once your fee structure is in place, it is important to clearly communicate it to patients, either through a fee schedule posted in your office or through patient handouts and information packets.
• Keep the complete amount of your fees for your exams and professional charges.
• The acquisition of equipment and administrative support services at their fair market value is a possibility, as long as state restrictions are met.

Qualifications and Experience of Eye Health Practitioner:

• A professional who holds a degree in Optometry (OD) or Ophthalmology (MD or DO) from an accredited institution.
• A legitimate license in the field of optometry or ophthalmology.
• General Liability Certificate.
• In the event that a professional makes an error, omission, or provides inadequate services to a client, the Professional Liability Insurance Certificate would come into play.
• W-9 Form.
