Qualifications and Experience of Optometry Practitioner:

• An individual who holds a degree in Optometry (OD) or Ophthalmology (MD or DO) from a certified college or university.
• A legitimate grant to conduct optometric or ophthalmologic services.
• General Liability Certificate.
• Securing a Professional Liability Insurance Certificate allows for peace of mind and confidence in the services provided to clients.
• W-9 Form.
