Qualifications and Experience of Vision Care Consultant:

• A practitioner who has obtained a degree in Optometry (OD) or Ophthalmology (MD or DO) from an accredited school.
• A recognized permit to practice optometry or ophthalmology.
• General Liability Certificate.
• A certificate for Professional Liability Insurance, also known as Errors and Omissions Insurance, is an essential document for those who provide professional services.
• W-9 Form.
