Qualifications and Experience of Optometric Practitioner:

• A medical professional who has successfully completed an accredited program in Optometry (OD) or Ophthalmology (MD or DO).
• Verification of licensing for optometric or ophthalmologic practice.
• General Liability Certificate.
• In the event that a professional makes an error, omission, or provides inadequate services to a client, the Professional Liability Insurance Certificate would come into play.
• W-9 Form.
