Qualifications and Experience of Vision Care Provider:

• A medical professional who has successfully completed an accredited program in Optometry (OD) or Ophthalmology (MD or DO).
• The possession of a current and lawful license to practice in optometry or ophthalmology.
• General Liability Certificate.
